According to USCIS, you must submit the $495 filing fee and supporting documents along with the following USCIS forms:
- Form I-821D (Consideration of DACA)
- Form I-765 (Application for Employment Authorization)
- Form I-765WS
If you would like to receive an email and/or text message notifying you that your application has been accepted, complete Form G-1145 (E-Notification of Application/Petition Acceptance) as well.
Please note that USCIS will reject DACA applications with outdated forms. Make sure that you have the most recent versions by checking the expiration date in the top-right corner of each form.
